Our social workers specialize in many aspects, including public health, palliative care, geriatric, and mental health care or inpatient medical care. They help patients & their families receive all the essential support and resources by coordinating with other healthcare professionals and organizations. They primarily assist the terminally-ill patient’s family members with discharge planning, counseling, case management, and referrals, among others. Generally, our social workers help patients & their family members navigate the medical system, assess and monitor their overall wellness, provide counseling and therapy sessions, and communicate the patient’s needs and concerns to a larger medical team.
